Exactly. Off-label selling can be done by pharma but they typically have
clinicians involved doing it directly with other clinicians. I used to work for pharma companies and we would employ physicians to talk with other physicians. It was very discrete and highly supervised by our medical, regulatory and legal departments. The point being the general public would not typically be aware of off-label drug use, and they would never see any promotion of it.
I work for a medical device company and our legal department frequently emphasizes aberrant sales and marketing practices are the biggest potential civil money penalties facing our company. It's a very quick way to get fired if you transgress here.
